JFS Patient Navigator Volunteers Make a Difference in the Community

PNB_malka_1.1-300x198The Massachusetts Healthy Aging Collaborative is a network of healthy aging leaders across the state who have come together to advance healthy aging.

Recently, the JFS Patient Navigator program was featured in their blog, under Stories from the Field.  In the post, Malka Young, LICSW, Director of Community Impacts at JFS, reflects on the many ways the program supports healthy aging in the community – both among frail older adults living in Metrowest as well as the skilled and dedicated volunteer Patient Navigators who help them access vital health services.
